Medical Form (to be verified by any MBBS doctor), Indemnity Bond from the website.  Both the forms along with Age Proof Certificate, ID Proof and Previous Course certificate (if applicable) are required to be uploaded while applying online. All the uploaded documents must be submitted in hardcopy at the time of reporting.


Fees must be paid online without which online application will not be submitted.


Seats are allotted only on the 'First Come First Serve' basis and admission to the course is rejected if:

   1. Any of the forms is incomplete or not verified 
   2. Identity certificate is not clear or not relevant.
